• Service bundle information is a non-personalized estimate of costs that may be incurred by the patient for anticipated services and those actual costs based on services provided to the patient must be provided.

• Patients has the right to request a Good Faith estimate from the center.

• There is no financial assistance policy.  Any bills unpaid by patient will be sent to a collection agency

• Anesthesia providers are contracted through Anesthesia Services Plus.  Please contact their billing office for a quote 1-800-437-5179.
